The Kaiser Permanente Bridge Program
Atlanta Coalition of Performing Arts (ACPA) is extremely pleased to partner with Kaiser Permanente to provide an affordable health care plan to eligible ACPA Individual members.

The Kaiser Permanente Bridge Program is uniquely designed to help those who are uninsured, income-eligible obtain health care coverage for a low monthly charge for up to 2 years. Eligible individuals pay a fraction of the premium cost and Kaiser Permanente subsidizes the rest. Coverage includes preventive services, hospitalization, comprehensive pharmacy and more.

ACPA Individual Member Eligibility Definitions
At least one person in the family must be a current, or join as an Individual Member of the Atlanta Coalition of Performing Arts in order to be eligible for the two-year Kaiser Bridge program. Eligibility is further defined as:

Current Individual Members are required to renew, or have recently renewed their membership for one (1) additional year.
Persons who are NOT current Individual Members of ACPA are required to, at time of application or before, join for a period of two (2) years. Annual membership is currently $50.
Paid staff of ACPA Member Organizations, members of Actors Equity or other entertainment unions (proof required) or students (valid student ID required) may join as Individual Members at a discount of $40

**Due to Limited Enrollment in the Kaiser Permanente Bridge Program, priority will be given to Current Individual Members of ACPA, paid staff of ACPA Member Organizations and members of Actors Equity or other entertainment unions (proof required).

Kaiser Permanente Bridge Program Eligibility Requirements
Please read carefully.

Applicants and all dependents must live in Kaiser Permanente's 28 county metro Atlanta service area
The annual household income must meet the Kaiser Permanente Bridge Program income guidelines (see guidelines below)
Applicants cannot be eligible for any other health insurance program including Medicare, Medicaid, PeachCare or employer-sponsored health care
At least one person in the family must be a client/member/student of one of our community partners (example: ACPA)
Must be age 64 and under
Applicant may not have been previously enrolled in the Kaiser Permanente Bridge Program
